Report an Incident

To inform the Office of Equity and Inclusion about incidents related to discrimination, harassment, retaliation, sexual assault, domestic violence or stalking, please use our secure 'Report an Incident' Online Form.

To report an emergency, please call the CSUDH Police Department by dialing 911 or (310) 243-3333.

In all cases, CSUDH strongly encourages students and employees to report sex discrimination, sexual harassment, sexual assault, sexual misconduct, dating and domestic violence, and stalking incidents to the university and to the police to ensure your safety and the safety of others.


Providing Notice to the University

You are encouraged to fill out the CSUDH Online Form, which provides information directly to the Office of Equity and Inclusion.

You can also set up a time to meet with a representative from the Office of Equity and Inclusion by contacting the office at (310) 243-3492.

Most CSUDH employees have a duty to report sex discrimination, sexual harassment, sexual misconduct, dating and domestic violence, and stalking incidents when they are on notice of them. When a survivor/victim tells the Office of Equity and Inclusion or another employee about a sexual misconduct, dating or domestic violence, or stalking incident, the victim has the right to expect the university to take immediate and appropriate steps to investigate what happened and to resolve the matter promptly and equitably. 


Reporting to the Police

CSUDH also encourages students and employees to report sexual misconduct, dating and domestic violence, and stalking to the CSUDH Police Department. You can call 24 hours a day to speak to a dispatcher, who can assist you in calling the officer on duty. The CSUDH Police Department is located at:
